C肝治療後的肝癌預測工具 (GES score)

GES (General Evaluation Score) 是一種簡單且經過驗證的計算工具，是由埃及Gamal Shiha團隊設計出來，用於預測慢性C型肝炎（HCV）患者在經過口服抗病毒藥物（DAAs）治療後，未來發生肝癌（HCC）的風險。此分數包含幾個指標，包括年齡、性別、血清白蛋白(Albumin)、甲型胎兒蛋白（AFP）以及肝纖維化階段。GES 分數將患者分為低、中、高風險組，並已在國際多中心成功驗證。此工具能有效協助臨床醫生進行個性化的 HCC 風險監測，提高監測效益並減少不必要的負擔​。不過現況在台灣，只要您有B型或C型肝炎，不管是否治療過，均可使用健保接受定期肝癌篩檢。

公式介紹 GES Score=(Age Factor)+(Gender Factor)+(Fibrosis Stage Factor)+(Albumin Level Factor)+(AFP Level Factor)

根據 GES 分數，風險結果可分為以下三組風險：

  • 低風險：≤ 6 分
  • 中風險：6–7.5 分
  • 高風險：> 7.5 分
GES Score 各變量對應的分數：
  1. 年齡 (Age Factor):
    • 年齡 ≤ 54 歲：0 分
    • 年齡 > 54 歲：1 分
  2. 性別 (Gender Factor):
    • 女性：0 分
    • 男性：3.5 分
  3. 肝纖維化階段 (Fibrosis Stage Factor):
    • F3：1.5 分
    • F4：3 分
  4. 白蛋白 (Albumin Level Factor):
    • 血清白蛋白 ≥ 3.8 g/dL：0 分
    • 血清白蛋白 < 3.8 g/dL：2 分
  5. 甲胎蛋白 (AFP Level Factor):
    • AFP ≤ 20 ng/mL：0 分
    • AFP > 20 ng/mL：3 分
